You can pay by Credit Card, E-check or using PayPal funds (see below if you would like to pay by mailing a Check or Money Order). Shipping and handling is included. Sales tax will be added for Arizona orders only. If you require larger quantities more than 10 please visit my standard order page so that your discount is calculated correctly.

Please note that in your shopping cart the quantity box will show "1" but the true quantity is located in the Item box. If you make a mistake, check the "Remove" button and the "Update Cart" button to start over.

Allow one to three weeks for delivery. If you have not received your sheets after one month, e-mail me to request order status. Occasionally packages get lost in the US mail or address labels are destroyed. I have filled thousands of orders over the years so I guarantee you will receive your order and you will be satisfied with it. I am not set up to provide shipping dates or tracking information but I do try to process orders at least every 1 - 2 weeks. I am a full time Anesthesiologist working in a very busy private practice so please realize my personal call and work schedule do not allow me to fill orders daily.
